The student pilot's failure to maintain directional control.
Factual narrative
On July 17, 1998, about 1207 eastern daylight time, a Cessna 150M, N704FE, registered to Ormond Beach Aviation, operating as a 14 CFR Part 91 instructional flight, veered off runway 26 after about 25 to 30 yards of takeoff roll at Ormond Beach Municipal Airport, Ormond Beach, Florida. Visual meteorological conditions prevailed and no flight plan was filed. The airplane received substantial damage, and the student pilot was not injured. The flight was originating at the time of the accident. The pilot stated that at the start of his takeoff roll on runway 26, the airplane veered to the left and, "...I was unable to stop it so, the plane ran onto the grass with the nose wheel collapsing." The student pilot lost directional control upon application of take-off power and ran off the left side of the runway, collapsing the nose gear and damaging the firewall and propeller.
