Factual narrative
COMPANY MAINTENANCE CREWS REPLACED THE HIGH PRESSURE BLEED VALVE PRIOR TO THE FLIGHT AND INSTALLED THE MOUNTING CLAMP IMPROPERLY. AFTER TAKEOFF THE VALVE DISCONNECTED AND THE BLEED AIR DISCHARGE ACTIVATED THE FIRE WARNING SYSTEM. THE CREW DISCHARGED BOTH FIRE BOTTLES TO NO AVAIL, LANDED AND EXECUTED AN EMERGENCY EVACUATION. DURING THE EVACUATION 14 PASSENGERS RECEIVED MINOR INJURIES. Source: NTSB Aviation Accident Database (Pre-2008 Archive) Retrieved: 2026-02-12
