THE INADEQUATE PRELIGHT PREPARATION BY THE PILOT IN HIS FAILURE TO ENSURE THE FILLER HOSE WAS REMOVED BEFORE ATTEMPTING TO TAKEOFF.
Factual narrative
THE PILOT WAS CONDUCTING CONTINUOUS AERIAL APPLICATION FLTS FROM A FIELD SITE, AND HAD LANDED TO REFUEL AND RELOAD CHEMICAL.
