Did anyone die from injuries sustained during the operation, or within 30 days of the event?

Q2 · Serious injury

Did anyone suffer a "serious injury" as defined by §830.2 — hospitalization >48 hr (started within 7 days), bone fracture except simple fingers/toes/nose, severe lacerations / nerve / muscle / tendon damage, internal organ injury, or 2nd/3rd-degree burns covering >5% of body?

Q3 · Substantial damage

Did the aircraft sustain "substantial damage" — damage or failure that adversely affects structural strength, performance, or flight characteristics, and would normally require major repair or replacement of the affected component? Excludes engine failure, damage limited to engine accessories, prop blade/tip damage, dents in skin, small holes, and ground damage to rotor/prop blades, landing gear, or wheel/tire/fairing.

Q4 · §830.5 immediate-notification triggers

Did the event involve any of the following? (select all that apply)

If reportable.

  1. Notify NTSB immediately by the most expeditious means available. Phone: +1-844-373-9922 or +1-202-314-6290 (24-hour duty officer).
  2. File NTSB Form 6120.1 within 10 days for accidents (within 7 days for an incident IF requested by NTSB).
  3. Preserve aircraft wreckage, mail, and cargo until NTSB / FAA releases it (§830.10).
